  • Be a planner: weddings, events, corporate. 

  • Be able to provide proof of legal business status or work for a company that can provide such proof. For example this could be your Business License, Certificate DBA proof, or EIN uploaded to this application, see attachment link below.

  • Be able to provide a currently valid certificate of General Liability Insurance, with coverage limits of at least $1,000,000 per occurrence and $2,000,000 in the aggregate, or work for a company that can provide such a certificate. Please upload a copy of your COI and Business Proof to this form.

Application One-Time Fee: $50/per person
Annual Fee: $199.00/per person
